The UMass Memorial Health - Health Risk Assessment is an educational tool for personal use only and made available to you at no charge. It is designed for adults 18 years and older living in the United States. It was developed using current national standard guidelines. It is not intended to diagnose, treat or prevent medical or other health conditions. If you have a medical emergency, call 911 immediately.
Your completion of the Health Risk Assessment is voluntary, and you may exit the assessment at any time. A personalized summary, created by Web MD, our Clinical Risk Assessment partner, of your results and suggested recommended actions will be ready within minutes after completing the questionnaire. Personal identification information is required to confirm patient identity, but your information will not be utilized for any other purpose other than as described in this document or sold. Your personalized summary will be displayed on a web page within the assessment, and you may choose to print the summary. The web page displaying your personalized summary will expire when you close your web browser and will not be retrievable. Please note, if you do not close your web browser, your personalized summary may be accessible by a subsequent user. You will also receive an e-mail with suggestions to improve your health and information should you choose to schedule an appointment with a provider.

Choice of Law and Venue
This agreement is entered into and performed in the Commonwealth of Massachusetts and you agree this agreement is governed by the laws of the Commonwealth of Massachusetts, without consideration to the principles of the conflicts of law. Your continued use of our Services confirms your consent and submission to personal jurisdiction in the Commonwealth of Massachusetts and venue of Worcester County for any state or federal action arising out of the use of our Services or these Terms. This agreement does not constitute doing business in any other state than the Commonwealth of Massachusetts. You agree that regardless of any statute or law to the contrary, any claim or cause of action arising out of or related to our Services, the Terms or the Privacy Policy must be filed within one year after such claim or cause of action arose or such claim or cause is barred.
